Integrated Behavioral Health Provider (BHC)

Licensed Clinical Social Worker (LCSW, LPC, LMFT)

We are seeking a skilled and compassionate Behavioral Health Consultant (BHC) to join our team at Praxis Health in Redmond, Oregon. As a BHC, you will play a crucial role in providing integrated behavioral health services within the Primary Care Behavioral Health (PCBH) model. This Full-time hybrid schedule position (combination of in-person and remote work) offers a unique opportunity to work with primary care providers to address our patients' behavioral health needs.

  • Integrated Behavioral Health & Primary Care

    Responsibilities:

  • Conduct evidence-based behavioral health assessments and interventions for patients in the primary care setting (in-person at via telehealth).

  • Operates in a consultative role within a primary care team utilizing the PCBH Model.

  • Collaborate closely with primary care providers to develop and implement integrated treatment plans that address both physical and behavioral health concerns.

  • Provides recommendations regarding behavioral interventions to the referring PCP

  • Conducts individual interventions with patients in context of interdisciplinary team including primary care and psychiatry.

  • Provide a combination of short-term and long-term, evidence-based behavioral health services to individuals, with opportunities to develop therapeutic and psychoeducational groups.

  • Offer support, education, and guidance to patients regarding behavioral health concerns, including depression, anxiety, stress management, general coping skills, lifestyle changes, and self-care strategies.

  • Participate in interdisciplinary team meetings to discuss patient cases, treatment planning, and coordination of care.

  • Maintain accurate and timely documentation of patient encounters in the electronic health record system.

  • Stay informed about current trends, research, and best practices in behavioral health and integrated care.

Minimum Qualifications

Education and/or Experience

  • Master's degree in Social Work, Psychology, Counseling, or related field.

  • Current licensure as a Licensed Clinical Social Worker (LCSW).

    Skills:

  • Experience working in an integrated care setting preferred, but not required.

  • Strong clinical assessment and intervention skills with a focus on brief, solution-focused therapy.

  • Ability to work collaboratively within a multidisciplinary team and communicate effectively with primary care providers.

  • Comfortable with using telehealth technology for remote patient care.

  • Flexible and adaptable to changing work environments and patient populations.

  • Commitment to providing culturally competent and trauma-informed care.

  • Will attend work regularly and on time, as scheduled
