Summary: As a member of a collaborative team, the Safe and Successful Youth Initiative (SSYI) Transitional Coach provides intensive intervention aligned with the wraparound philosophy and process to help proven risk youth comply with program participation and completion; helps plan, guide, and implement a program of comprehensive prevention and intervention services, and overall evaluation of program participants? needs. The SSYI Transitional Coach uses best practices that are family-driven, youth-focused, and strength-based; works with youth and families to create individualized strength-based interventions that include informal, natural supports as well as formal services aimed at achieving positive youth outcomes, safe and healthy behavior, and expand their sense of possibility and opportunity. SSYI serves court involved youth between the ages of 17-24 who may be incarcerated, and/or recently released from incarceration. The role of an SSYI Transitional Coach centers around case managing youth to reduce the likelihood of reoffending when in the community. Desired Knowledge and Qualification: Familiarity with the New Bedford community and its surrounding towns/cities. Understand and relate well to the target youth population/participants. Case Management experience. Knowledge of and ability to work collaboratively with local agencies and institutions that serve this population (i.e., social services, DYS, DCF, law enforcement, courts, parole, corrections, and others). Possess strong communication and interpersonal skills. Able to prioritize tasks, multi-task when necessary, and work independently in the absence of supervision. Experience with Microsoft Office. First Aid/CPR Certified Requirements: 1. Minimum education and experience: (a) Minimum High School Graduate or Equivalent, Post HS education preferred. (b) Three years of counseling, social work, or related experience, one year of which must have included work with youth considered at high risk of school failure. 2. Licenses and certifications: ? Valid driver?s license and access to a registered and insured vehicle with the following minimum amounts of liability coverage: \$100,000 injury per person; \$300,000 injury per accident; and \$5,000 property damage. ? Satisfactory criminal history background checks as determined by the agency.
