MACON PROGRAM FOR PROGRESS INC Healthy Opportunities Specialist in Franklin, North Carolina
POSITION: Healthy Opportunities Specialist STATUS: FT/ Days **PAY: $14.25 - 17/hour
JOB SUMMARY: The HOP Specialist position is a vital position in our Community Services Department. This position can range from delivering food box to a family in need, to acting as project manager for a home repairs. Though the duties may vary, this position works hard to assist the community and those in need.
RESPONSIBILITIES: Plans, and develops, coordinates, and implements the systems, services, and partnerships that ensure a well-functioning system of eligibility determination, recruitment, selection, enrollment is provided; and that the services provided within this system comply with regulatory standards following policies, procedures, and the mission, vision, and values of MPP.
Essential Duties:
- Ensures Medicaid eligibility criteria for participants and works to link them to the correct services, ensuring a smooth transition of dual-enrollment and service distribution.
- Maintains eligibility determination records for each participant who is currently enrolled and for one year after the participant has either stopped receiving services or is no longer enrolled.
- Establishes and maintains a volunteer database, creating a working relationship with volunteers, groups and provides community outreach to educate individuals about volunteering opportunities at MPP for the HOP program.
- Handles HOP invoicing, filing, paperwork and works directly with contractors, staff and participants.
- Maintains all HOP lists and documentation on an ongoing basis.
- Coordinates and implements a system of recording, monitoring and analyzing information regarding participant information and satisfaction.
- Works with the HOP Manager to develop bi-annual and annual reports.
- Responsible for all data entry, tracking and submission for the HOP program.
Education and Experience Requirements:
- Associate of Arts degree, AND
- 1to 2years related experience and/or training,
- OR,equivalent combination of education and experience
- Ability to work both independently and as a contributing team member.
- Proficient oral and written communication skills.
- Working knowledge of Microsoft Office Products.
