R.A.I.N. Total Care Inc. OAC Case Management Pilot Case Manager in Bronx, New York

Position Summary: Responsible & accountable for ensuring achievement & excellence in the service delivery of the Case Management Pilot Program through assigned caseload. The Case Manager is responsible for understanding R.A.I.N. policies & procedures, CFPA Community Funding Project-Administration on Aging Performance Standards to perform the job duties. This position requires an engaging Case Manager w/a case management background & is knowledge of the community demographics & characteristics.

Essential Duties & Responsibilities: - Provide direct services to clients, including intake, referrals, benefits & entitlements assistance, advocacy, counseling & follow-up; crisis intervention, home visiting & wrap-around training. - Maintain & update clients' files & ensure that all information is documented, & all contractually required documents are maintained in files. - Fill out entitlements/benefits applications such as SNAP, Medicaid, Medicare Savings Program, Senior Citizen Rent Increase Exemption, Home Energy Assistance Program, NYC-210, IT-214, Housing applications, etc. - Conduct recurring & event-based in-home & office assessments to determine client needs & concerns, so as to evaluate initial & ongoing program eligibility. - Assess & regularly track changes to the clients cognitive & functional limitations, as well as family circumstances, financial resources, informal/formal support status, home environment, & any additional factors linked to the clients safety & ability to carry out activities of daily living. - Develop care plans w/the client &/or his/her caregiver(s) to address the clients short & long-term needs. - Provide accurate & appropriate information on services, benefits, & entitlements that address the clients needs. - Arrange for provision of appropriate services for the client (as established in the clients care plan) & advocate/intervene w/other agencies on the clients behalf. - Provide crisis intervention & goal-oriented counseling to client &/or caregiver(s). - Maintain complete, detailed & accurate case records & ensure database is updated daily. - Conduct outreach events to engage target population for programs catchment area & know how to access interpreter services when needed. - Knowledgeable about & sensitive to socioeconomic, health care & other issues of minority & immigrant & veterans population. - Sensitive to issues of gender identity & sexual orientation. - Knowledgeable about linguistically & culturally competent service providers in the community - Knowledgeable about the communication needs of persons w/visual &/or hearing impairments. - Comply w/funding source regulations & participate in company training & assigned training. - Represent agency in the community & in interagency & intracompany activities. - Support cultural diversity by ensuring that the delivery of quality, equitable & culturally competent client-centered services are provided; promoting & maintaining an inclusive work environment & culture that is respectful & accepting of diversity. - Participate in outreach strategies to engage target population & community service providers to support client needs & support.

Minimum Qualifications: - BSW or related Bachelors level degree (e.g., social services, public admin., nursing or public health); w/at least three years more of casework experience in a community service or social action program - Exp. working in the field of aging - Crisis-management skills - Computer skills essential (Microsoft Office) preferred - Ability to develop & maintain effective working relationships w/other staff, volunteers & clients - Competency in data entry & documentation preferred
