The University of Maryland School of Pharmacy, Department of Pharmacy Practice and Science has an open position for anAdvanced Practice Pharmacist located at Spring Grove Hospital Center in Catonsville, MD. *The Spring Grove Hospital Center is Maryland's largest state operated psychiatric facility. The hospital serves individuals involuntarily committed as well as forensic cases. Established in 1841, the University of Maryland School of Pharmacy is the fourth oldest school of pharmacy in the United States. Responsibilities include administration and/or supervision of staff or drug utilization reporting. * Reviews, analyzes, and interprets patient information including health records, medical forms, laboratory work, and records of past medication use. Analyzes drug usage trends in large databases. * Actively monitors medication use in individual patients and populations. * Interacts with providers and other health professionals regarding appropriate medication use, medication safety, and pharmacotherapeutic monitoring. * Documents consultations and prescriber interactions, creates reports that summarize clinical activity and presents them to supervisors, reviewers, and oversight panels. * Coordinates activities with officials from state agencies and/or state-appointed vendors to facilitate patient access to appropriate medication. * Performs formulary and non-formulary reviews as assigned. Performs adverse drug reaction (ADR) reviews.Performs medication event reviews. * Attends rounds or treatment team meetings. * Provides quality improvement services necessary for accreditation, certification or maintenance of permits and licenses. * Performs administrative duties including but not limited to scheduling and coordination of services. * Precepts pharmacy students and residents. * Reviews patient medication profiles to dispense and distribute medication. * Participates in collaborative drug therapy management. * Attends and participates in pharmacy and therapeutics (P & T), pharmacy & nursing (P & N), patient care, Patient and Family Education, Forensic Review Board (FRB) and Seclusion and Restraint Reduction committees. * Participates in medication management through collaborative drug therapy management. Performs consults on requests. Performs discharge counseling for appropriate patients.Performs medication administration record (MAR) reviews, Leads monthly medication education session for patients. * Instructor of Nursing Orientation, medical student lectures. Mentor psychiatric resident rotation. * Conducts medication use evaluations (MUE). * Provides articles for the pharmacy newsletter. * Available for drug information requests. * Prepares medications by reviewing and interpreting physician orders. Detect therapeutic incompatibilities. Dispense medication by compounding, packaging, and labeling pharmaceuticals. Writes renewal medication orders, and lab orders required for patients taking specific drugs per physician's orders. * Performs other duties as assigned. MINIMUM QUALIFICATIONS: Education: *Bachelor's degree in pharmacy. Doctoral degree in pharmacy (Pharm.D) preferred. *Experience: *Two (2) years of experience in a pharmacy, OR One (1) year post-baccalaureate or post-doctoral residency plus one (1) year of experience, OR Two (2) years post-baccalaureate or post-doctoral residency, OR Certification in a relevant specialty by the Board of Pharmaceutical Specialties, such as psychiatry, or Commission for Certification in Geriatric Pharmacy. Preferred Experience:Direct patient management, collaborative drug therapy management or prescriptive authority experience preferred. *Supervisory Experience: *N/A. *Certification/Licensure: *Board Certified Psychiatric Pharmacist (BCPP) or Board Certified Geriatric Pharmacist (BCGP) or eligibility preferred. *Other: *Candidates selected for employment may be subject to medical inquiries and/or medical examinations to determine ability to perform the job. Employees in this job class may be subject to pre-employment and random drug testing. *KNOWLEDGE, SKILLS, AND ABILITIES: * Comprehensive knowledge of pharmaceuticals, their indications, interactions and potential side effects. * Knowledge and understanding of relevant physiology, pathophysiology and disease states. * Skill in the assessment of patients and their drug regiments. * Skill in the development of pharmacotherapeutic plans. * Skill in teaching health professional students and residents. * Skill in communicating with people of all levels. * Excellent organization and time management skills. * Skill in use of relevant PC applications including MS Word and Excel and relevant library databases including Medline. * Knowledge of relevant accreditation and certification requirements.
